Private visa (visiting relatives/friends)
1. Passport + copies (residence permit if not Lebanese + copy)
Passport should meet the following requirements:
Be valid for at least three months after the stated date of departure from Ukraine;
Have at least two blank visa designated pages;
For minors - + copies of parents’ passport
Copies of all previous (valid and expired) visas and used pages must be made.
2. Completed visa application form from the website http://visa.mfa.gov.ua
Visa application forms of minors shall be submitted for each person
3. Photo
Photo must be in color, 35x45 mm, printed on normal photographic paper, taken against a light background; full face taken straight forward; the person is to be captured with neutral facial expression, with his/her eyes open and mouth closed. The photograph of a person, who for religious or medical reasons is wearing a head cover, must show the person's cheeks, chin and forehead.
4. Valid health insurance with coverage of at least 30,000 EUR or equivalent in other currency. Holders of diplomatic or service passport shall be exempt from this requirement;
5. Bank account for the last 3 months (at the rate of 1000 USD per 15 days stay in Ukraine)
Bank account must be originally stamped. Holders of diplomatic or service passport shall be exempt from this requirement;
6. Employment letter/Student/commercial circular/ID for engineers/pharmacists/attorneys etc.
7. Family register (issued not later than 6 months before the applying date)
8. Notary invitation from the inviting person.
It must be issued by Ukrainian notary.
MANDATORY requirements for the invitation:
Information of the inviting person - name, surname, passport data (series, number, date of issue, issuing authority) and a permanent or temporary residence (if not Ukrainian), address of residence in Ukraine, mobile phone, the commitment of an inviting person to cover all expenses of invited person during his/her stay in Ukraine and his/her departure from Ukraine
Information of the invited person - name, surname, information about its date and place of birth, nationality, passport data (series, number, date of issue, issuing authority), address in Lebanon, purpose of the trip (private), the period of the planned visit to Ukraine, the number of entries and the place of his/her residence in Ukraine.
9. Copies of internal Ukrainian passport / residence permit (if not Ukrainian) of the inviting person – all pages with marks and stamps needed!
Documents in Arabic must be translated into Ukrainian or English

Business visa

MANDATORY requirements for the invitation:
shall be issued on the official letterhead/blank/form of a legal entity, must contain the number in the Unified State Register of Legal Entities (ЄДРПОУ), registration number, date and reference number, signed by the CEO / owner (their deputies) of the legal entity, a mobile phone number of a contact person, the commitment of an inviting entity to cover all possible expenses of the invited person during his/her stay in Ukraine and his/her departure from Ukraine.
Information of the invited person - name, surname, information about its date and place of birth, nationality, passport data (series, number, date of issue, issuing authority), address in Lebanon, the period of the planned visit to Ukraine, the number of entries and the place where he/she will live during his trip to Ukraine.
9. Extract from the Unified State Register of Legal Entities, Individual Entrepreneurs and Public Associations of the employer of the inviting party

Student visa

Abovementioned documents must be legalized at the Ministry of Foreign Affairs of Lebanon and translated ONLY! into Ukrainian!
